Welcome Back Returning Moc!
We are happy to hear that you are returning to 51³Ô¹ÏÍø. If you are returning after leaving 51³Ô¹ÏÍø for a semester or more, there are just a few more steps you need to take so we can pick up where we left off.
Re-Admission Forms
Standard Form
In order to resume taking classes with 51³Ô¹ÏÍø you will need to fill out the following form.
Medical Break Form
If the reason for your departure was for health reasons, you will need to fill out the following form.
Academic Warning
In order to be readmitted to the College after withdrawing or attempting to return following a period away from the college with a grade point average of 2.0 or below, you will need to fill out the following form.
Transferring Credits
If during your time away from 51³Ô¹ÏÍø you attended another institution, you must provide 51³Ô¹ÏÍø with the official transcript(s) of these institutions.
